Office Updates Relating to COVID-19

Please review the critical information regarding our reopening below:

EVERY individual MUST wear a mask in order to enter our office, and their temperature will be taken before entering the operatory.

To help reduce the amount of people in our waiting room, only ONE adult may accompany a child to their appointment.  We ask that the parent wait in our waiting room, or in their car during your child’s appointment, only the patient will be allowed in the operatory for the safety of our patients, and employees.

In accordance with the CDC, ADA, CDA recommendations, we will be screening patients before they enter our office.  You will be required to fill out a form upon entering our office, and if you can answer yes to any of the questions your appointment must be rescheduled.

1) Has the patient/family member, or anyone you’ve been around who has been sick within the last two weeks with fever, cough, tiredness, shortness of breath, or any other symptoms?

2) Has anyone in the family traveled outside of Colorado within the last 4 weeks? If so, to where?

If you had an appointment between March 20th & April 24th while we were closed, please contact our office to reschedule that appointment.
